Referral pathway for GP’s (MHCP)
A referral letter is all that’s required to refer a patient to us under a MHCP.
Please ensure the MHCP referral contains the details here:
- Patient’s name
- Patient DOB
- Patient address
- Patient’s Medicare number
- Patient’s diagnosis or symptoms
- Number of therapy sessions needed
- GP practice details (inc contact details)
- GP Medicare provider number

We will follow up with the GP or GP’s clinic for clarification in the case that we receive a referral that is incomplete or invalid. Once a a valid referral is received, the GP will receive a referral confirmation letter from Jimmy after the patient attends their first session.
We can be available to meet with your Practice Manager to facilitate the referral process and provide information on the services we provide.
